Enrollment Forms and Liability Waiver

Parent’s must complete this form on behalf or with their child. Completed for each individual learner. Forms are due one week prior to the start of lessons.

Liability Waver is to be completed by new and returning families. It is also due prior to lessons and lessons are unable to start without it completed.

Participant must complete form prior to lessons. Adult’s are considered 19+ for this form. Forms are due one week prior to lessons.
